| Appointment Information For more information or to set up an appointment please contact the Central Scheduling Office at (410) 955-9270. The Johns Hopkins Comprehensive Diabetes Center is open Monday through Friday from 7:30 - 4:00. We are located on the 2nd floor of Johns Hopkins Outpatient Center. When arriving for your appointment be sure to bring with you your glucometer, list of prescriptions, referral (if you have one), insurance card and orange hospital card. If you have any question about your visit please call the Diabetes Center at (410) 955-7139. Also for any new patients we have put our forms online so if you would like to print and fill out the forms before coming to the hospital click the appropriate link for the person you are seeing. Dr. Saudek's, Dr. Golden's Dr. Clark's (Endocrinologists) new patient forms. We have also made available referral forms for our dietitian. You can download and print them by clicking Here.
